Patient Care Liaison Job Responsibilities
- Be responsible for assisting the Patient Care Coordinators and Client Care Managers with intake calls.
- Accurately enter new cases information into the database.
- Schedule patient care services.
- Conducts introduction calls to new members as well as customer service calls.
- Monitor/Navigate our database systems.

Qualifications
- High School Degree or equivalent required
- Home Health experience or previous Patient Care Coordinating experience in Home Health
- Bilingual Spanish/English required
- Must be computer savvy
