MD Revolution (MDR) is an innovative, rapid growth, Virtual Care Management company that uniquely combines technology, services, and analytics to deliver scalable, digital care to patients. We create and manage patient care programs on behalf of hospitals and clinics to deliver Medicare’s Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs, as well as Behavioral Health Integration (BHI), population health programs, Annual Wellness Visits (AWV), and other digital health services.

MD Revolution is looking for remote Virtual Medical Assistants to play a vital role in this new model of patient care delivery, by closely monitoring important health data of our patient population and communicating with, and caring for, patients telephonically and on our digital care platform, RevUp. As a Virtual Medical Assistant, you will have a fundamental role in direct patient care. You can make a difference in the patients’ care by connecting with them monthly to provide support and assist the patient to take charge of their health. Each month you can follow up on patient progress and continue to provide education and support to guide them in preventative care and improve their overall health. Use your medical experience and education to build rapport with our patients and help coordinate their healthcare.

Job responsibilities:

· Make up to 100 outbound call attempts per day to communicate with patients to discuss their overall health and wellness

· Communicate with patients via direct messaging, and online portal to discuss patient’s overall health and wellness

· Guide patients through their personalized health program, while collaborating with other members of the healthcare team. Establish goals and create an achievable care plan

· Review the patients’ plan of care with them, and assist, as needed, with care coordination such as appointments, medication refills, etc.

· Work within scope of practice with direct report to clinical leadership team

· Evaluate, monitor, and teach patients to develop self-awareness with data obtained from mobile health kit devices and mobile health apps

· Recognize situations that require the immediate attention of a provider and initiate escalation procedures when necessary

· Learn and effectively use our RevUp software platform to deliver patient care

Requirements:

· Current, active Medical Assistant or Certificate of completion of a Medical Assistant training program, or equivalent training and experience

· Proficient with using Mac

· 1-2 years patient care experience

· Exceptional interpersonal, organization and communication skills required

· Compassion and empathy for our patients. Establish and maintain a high level of trust while fostering feedback and collaboration. Comfortable leading one-on-one conversations with patients

· Ability to work virtually with teammates and supervisors

· Able to thrive in a fast-paced work environment

· Strong sense of ownership and initiative

· Proficiency with software programs including Microsoft Word, Excel, Outlook, and Apple hardware

· Spanish speaking a plus

· 40-hour work week

Job Type: Full-time

Pay: $15.00 - $17.00 per hour
