Overview
The Vice President of New York City Health Home reports to the Chief Operations Officer, CINQ-NY
HH IPA, LLC, with accountability for aiding the case management practice efforts for Cinq Care Inc. The
Vice President of New York City Health Home should embody CINQ’s core values, including

Primary Responsibilities
The Vice President of New York City Health Home will have the following responsibilities:
- Establishes, implements, and communicates the strategic direction of the organizations
operations division.
- Collaborates with executive leadership to develop and meet company goals while supplying
expertise and guidance on operations and systems.
- Collaborates with other divisions and departments to carry out the organizations goals and
objectives.
- Identifies, recommends, and implements new processes, technologies, and systems to improve
and streamline organizational processes and use of resources and materials.
• Ensures that departmental decisions and plans such as those for staffing, development,
organization, material efficiency, hardware acquisitions, and facilities are in line with the
organizations business plan and vision.
- Establishes, communicates, and implements operations-related New York State Health Home
policies, practices, standards, and security measures to ensure effective and consistent support
and execution.
- Reviews and approves staffing requirements for operations.
- Administers the budget and manages Profit & Loss Statement to ensure company’s goals are
being achieved.
- Responsible of metrics around growth, quality, and compliance. Report performance to the chief
operations officer and other leadership.
• Maintains knowledge of emerging technologies and trends in operations management.
- Identifies training needs and ensures proper training is developed and provided.
• Responsible for maintaining high employee satisfaction with all direct reports, their people and
to assist Operations Managers.
- Responsible for the execution of the Company’s quarterly, annual, and long-term goals in line with the Company’s Vision, Mission and Guiding Principles.
- Performs other related duties as assigned.
- Collaboration: The Vice President of New York City Health Home will work closely with other
business divisions to learn their needs, internalize their knowledge, and assist with solutions to
achieve the business objectives of CINQ Care Inc.
- Knowledge: The Vice President of New York City Health Home requires strong interpersonal
communication skills, excellent verbal. written, presentation, skills. Ability required to
interact with multiple facilities, staff, and community partners
- Culture: The Vice President of New York City Health Home is accountable for creating a
productive, collaborative, safe and inclusive work environment as part of the larger Company.
- Communication. Excellent verbal, written communication, and presentation skills; ability to
clearly articulate and present concepts and models in an accessible manner to CINQ’s team,
investors, partners, and other stakeholders. Proficiency in all Microsoft Office applications;

external constituents; and,

grow in a fast-paced, demanding, and entrepreneurial Company.

care to Black and Brown populations. This position is accountable for ensuring CINQ is positioned
to innovatively deliver on its promise;
Qualifications:
The Vice President of New York City Health Home should have the following qualification:
• Bachelor’s degree in social work, public health or related field: Masters preferred
• 5+ years in Care Management/Health Home experience • 5+ Supervisory / performance management experience
• Experience organizing and facilitating meetings and trainings
• Experience in community networking and relationship building
• Experience maintaining databases to track information and generate reports
- Proficient in the use of technology
- Understanding of payer requirements and reimbursements in a health care environment
- Computer literacy with Windows-based operating systems and MS Office applications (Word,
Excel, Outlook, PowerPoint)
- 5+ years of electronic health records experience
- Experience in managing P&L
- Experience in development and growth of workforce
- Experience in marketing development needs for operations
