QHSP Job at Garden Center Services
This position provides QHSP support to 3 CILA homes, and offers a flexible schedule (days, evenings and weekends). It requires 24/7 on-call cell phone availability for medical, behavioral, and other emergency situations.
Specific duties include but are not limited to:
- Coordinate all services including medical, behavioral, psychiatry, and other therapies for the individuals on your caseload
- Develop implementation strategies
- Provide guidance and direction to the CILA team leads and DSPs in the goals, objectives, data collection, and effective implementation of each individual’s annual, semi-annual, and behavior plans
- Prepare monthly progress notes for each consumer
- Maintain required documentation
- Maintain communication with guardians and family members
- Oversee consumer finances
- Other duties and responsibilities as necessary and assigned
Basic requirements
- Bachelor's Degree in human services field of study required
- One year experience working with individuals with developmental disabilities required
- Effective communication skills, both written and oral
- Demonstrated ability to manage multiple priorities
- Prior experience in a residential setting, preferably in a supervisory role
- Valid driver's license with a good driving record and current insurance

Pay: $45,000.00 per year
