The Program Coordinator is responsible for providing outreach and administrative tasks for the MHL Program. Tasks include data entry, outreach phone calls to members, scheduling member assessments, data spreadsheet tracking based on reporting requirements and other duties as assigned. Communicate and collaborate with Care Coordinators from Integrated Care Organizations (ICO), internal department team members, all AAA1B departments, Vendor Agencies and community resources.


JOB DESCRIPTION:

Under the general supervision of the Clinical Manager, the Program Coordinator:

  • Provides administrative support to Supports Coordinators, and Clinical Managers, such as: scheduling appointments, entering participant data into database software, maintaining participant files, completing participant documentation, maintaining office supplies, training on IT systems, and coordinating work product with the Service and Authorization Team.
  • Conducts audits and informs Clinical Manager when information in files are incomplete or missing.
  • Monitors Outlook emails and Harmony on an hourly basis to receive requests from Supports Coordinators; respond to requests from Managed Care Organizations (MCO); and processes information/requests to meet all program requirements. PC also is responsible for data entry into the MCOs Electronic Medical Record database (EMR) online system.
  • Coordinates and processes all necessary participant paperwork related to eligibility, status forms, service arranging, billing and other Supports Coordinator activities.

Minimum requirements are:

  • Associates Degree in Liberal Arts, or Health/Human Services preferred. High School Diploma or GED minimum requirement.

  • 1-2 yrs. experience preferred in: office administration, health/human services or related field. 6 months 1 year minimum required

  • Excellent written, verbal communication, organization, interpersonal and problem-solving skills

  • Strong computer and data entry skills (40 wpm) in Word, Excel and Outlook required
