Salary Range: $130,000.00 to $260,000.00 per year

By adhering to Connecticut State Law, pay ranges are posted. The pay rate will vary based on various factors including but not limited to experience, skills, knowledge of position and comparison to others who are already in this role within the company.

Physicians Alliance of Connecticut (PACT, LLC)is a multi-specialty physician group that is in partnership with Hartford Healthcare is seeking a full time or part time Physicianto work with our PACT Primary Care in Hamden. The position is located in the Hamden office along with skilled nursing facilities. The office hours are Monday through Friday 7:45am to 5:00pm and one monthlySaturday morning.


Summary

Under indirect supervision, provides and manages health care for a specified patient population.



Essential Functions

Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population.
Engages patients and their families or caregivers in understanding, setting and monitoring selfmanagement care plans, and documents individualized care plans and care coordination in the patient chart.
Analyze patients medical history, medication allergies, physical condition and examination to determine best care
Diagnose medical condition
Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
Refers patients to specialists or to internal medicine as appropriate.
Directs and coordinates the patient care activities of nursing and support staff as required.
Respect and promote patient rights and privacy.
Responding appropriately to emergency situations.
Participate in meetings to evaluate practice processes and work flows pertaining to patient care and quality improvement efforts.
Participate in specified health promotion, education and/or prevention programs.
Participates in population health activities as directed by the practice.
Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards


Skills and Knowledge

Knowledge of legal and ethical standards for the delivery of medical care.
Understanding of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosage and instruct patients in correct usage.
Knowledge of current principles, methods and procedures for the delivery of medical evaluations, diagnosis and treatments in area of expertise.
Demonstrated skill in interviewing and patient coaching.
EHR documentation as it pertains to patients and populations of patients.

Education and Experience

Medical Doctor with 3 to 5 years of direct patient experience which may include residency.
Medical License, Board certification, Federal DEA certification
