We are seeking a Mental Health Outreach Worker :to provide street outreach and linkages to homeless individuals in the Napa county area. The Mental Health Outreach Worker engages and builds rapport with people exhibiting more acute mental health symptoms. Additionally, they assess for behavioral healthcare and medical needs and then assist to coordinate care and navigate through potential barriers to the participant’s goals and/or well-being. The position helps clients by connecting them to Napa mental health resources, and working closely with other staff to support client needs.

Responsibilities: :
    • Crisis de-escalation in the field and in the shelter.
    • Assist with client intakes, VI-SPDATs, and other essential client services.
    • Complete assessments for referral to the Coordinated Entry System to link people to community and county health services, housing programs, and medical services
    • Assess substance use disorders and implement harm reduction strategies as well as connect people who desire to move to abstinence to those services
    • Move people with higher acuity and special needs towards engagement and the accomplishment of their goals
    • Connect participants to primary healthcare, food, shelter, services for mental health, housing, housing documents (ID, SS card, etc), substance recovery, physical health care, benefits, employment, advocacy, and other essential services.
    • Utilize motivational interviewing techniques to explore participants’ motivation towards behavioral change

Qualifications: :
  • BSW or BA in applicable field of study.
    • Must possess 1 year of field experience working with people with serious mental illness, individuals with a dual diagnosis and/or the homeless population.
    • Bilingual Spanish or other language preferred.
    • Must have a working knowledge of Psychiatric Disorders as well as knowledge and ability to implement the following evidence-based models: Harm Reduction, Strength-based Case Management and Motivational Interviewing.
    • Reliable transportation and proof of a valid and current California Driver’s License and current insurance along with a clean DMV record required.
