As a Medical Social Worker, you will be an integral part of our interdisciplinary team, working alongside physicians, nurses, and other healthcare professionals. Together, we create personalized care plans that address the physical, emotional, and psychosocial needs of our patients and their families. In this role, you will provide essential support and guidance to patients and their families during the end-of-life period. Your responsibilities will include assessing the psycho-social and emotional needs of patients, coordinating care with the interdisciplinary team, offering counseling and psychotherapy, and providing crisis intervention when necessary. You will also educate patients and families about their treatment plans and assist in making end-of-life plans. Our electronic medical records system allows you to complete all charting inside the patient's home.
Duties include:

• Coordinating care with the Hospice Interdisciplinary team
• Conducting psycho-social assessments of patients and families
• Providing counseling and psychotherapy to support emotional well-being
• Offering crisis intervention during challenging situations
• Educating patients and families about hospice and palliative care processes
• Documenting and reporting patient information accurately
• Assisting with end-of-life planning and discharge planning
• Participating in in-service education programs

Qualifications for this position include:

• Graduate of an accredited school approved by the Council on Social Work Education
• Master's degree from a school accredited or approved by the Council on Social Work Education
• Minimum of one (1) year of experience as a Medical Social Worker
• Good physical stamina and mental health, including negative TB skin tests and/or CXR
• Ability to read and follow written instructions and document care given
• Self-directed with the ability to work independently
• Current CPR certification
• Flexibility and willingness to fulfill role obligations
• Demonstrates tact, patience, and good personal hygiene
• Valid driver's license and insured automobile as per Lorian Health requirements
• Proficient verbal communication skills in the language of the client and excellent documentation abilities
