If you are located in Jacksonville, FL, you will have the flexibility to work remotely* as you take on some tough challenges.

Primary Responsibilities:

• Assesses members and family psychosocial needs and develops plan of care in concert with members, providers and other discipline that impacts member’s health
• Advises members on practical ways in which they can improve their health by incorporating lifestyle and dietary changed into their daily lives
• Educates members on appropriate diet and nutrition for individually specific needs
• Links members and family to appropriate community resources including but not limited to, information/referral to sources of financial assistance, transportation, support groups, and other community services
• Documents patient/family status, diagnosis, treatment plan, goals, and interventions, evaluation results, observations, and progress in clinical platform
• Educate members and families to help simplify and navigate the healthcare system
• Serves as patient advocate and liaison with physicians, families, insurance company, community agencies and others as needed to ensure members’ access to health benefits

Required Qualifications:

• Bachelor’s degree in a nutrition -related major
• 2+ years of related experience and /or training
• Ability to interact and communicate with members in a virtual/telephonic setting
• Proficient computer skills including Microsoft Office, email and web-based application
• Demonstrate clear oral and written communication skills
  

Preferred Qualifications:

• Registered Dietician (RD), Registered Dietitian Nutritionist (RDN), Certified Nutrition Specialist (CNS)
• Bilingual Spanish
• Background in Managed Care
• Ability to trouble shoot, analyze situations and make appropriate decisions
• Ability to self-direct and manage time
• Demonstrate knowledge of principles of patient education
• Ability to handle multiple tasks simultaneously
