Family Facilitator, Santa Barbara Job at Casa Pacifica
Wage band range $21.00 to $33.00 per hour
Starting pay is typically between $25.00 to $28.00 per hour; however, it is ultimately determined by contract obligations, the scope of the position, the candidate's relevant experience, and internal equity. Bilingual in Spanish, preferred and position is eligible to test for a bilingual pay incentive. This position will not be offered as a remote workstyle.

Come join our team of supportive team members who coordinates the development and implementation of individualized service plans and provides direct services to children and their families in order to maximize the involvement of all persons and the implementation of plans and facilitate the integration, coordination, and implementation of services.
DUTIES AND RESPONSIBILITIES:
- Experience engaging parents as partners and demonstrated ability to work with families from diverse ethnic and cultural groups successfully.
- Assist in the ongoing development and evaluation of the service program.
- Provide leadership in assessing, developing, implementing, and evaluating individual wraparound plans.
- Assume responsibility for configuring and facilitating child and family teams.
- Coordinate, schedule, and facilitate child and family team meetings for the development of individualized wraparound plans.
- Ensure all required assessment measures are completed promptly and within the required timeframes.
- Assume responsibility for the Family Service Plan management activities and implementation of services/coordination of resources for children and families.
- If required, conduct Medi-Cal comprehensive assessments and treatment plans for all assigned cases.
- Assist in monitoring the Child and Family Plan expenses/budget and secure community team approval for overall budgeted plans.
- Monitor and support the provision of unconditional care by reassessing and “redoing” individual plans as needed, ensuring that families have access to resources, and maintaining effective relationships with child and family team members.
- Provide crisis management to families and child and family teams when plans disrupt and onsite consultation to others involved in plans.
- Participate in 24-hour on-call system, including rotating weekend coverage.
- Attend Case Reviews on time and arrive prepared to discuss critical issues in families engaged in the process. Additionally, participates in individual clinical supervision.
- Carry a caseload averaging 8-10 families and is flexible in providing services countywide in various environments (home, school, community, juvenile hall, etc.). The caseload could temporarily be less than or greater based on program needs.
- Serve as a role model and advocate for children and families in wraparound.
EDUCATION and/or EXPERIENCE:
- Master’s degree with two years of experience working with seriously disturbed/at-risk children, adolescents, and their families.
- Experience in crisis management is a must.
- Trainee in an MFT/MSW/LPCC approved program or AMFT/ASW/APCC.
