Essential Job Functions:
Demonstrates effective management/leadership skills in the area of primary responsibility and in all facility activities.
- Displays a desire and ability to set and meet challenging objectives to find better or more efficient ways to do things and to compete against a self-defined standard of excellence. Displays team leadership skills to develop cooperation and teamwork by seeking input of group members, encourage participation, keep them informed and find solutions that fairly and equitably benefit most parties. Display initiative to act independently by taking effective action before being directed by supervisor or forced by events.
- Displays an ability to maintain consistent and stable work performance during stressful situations. Displays an ability to appropriately express own opinions relating to daily facility operations and to responsibilities assigned. Support the final decision of the facility and the Executive Director.
- Attends daily management meetings to provide information about prospective residents being followed and contacts made. Provides up to date admissions/discharges and census information to the Executive Director and Department Mangers. Consistently updates admissions, discharges, etc., for accurate census reports.
- Maintains confidentiality of facility and resident information at all times, as observed by the Executive Director.
- Keeps abreast of all competitive facilities in the market area and any changes that may influence facilities programs and progress. Based on identified market and community needs, recommends and assists in the implementation of two or more new programs or services annually.
Acts as liaison between residents/families and facility to answer questions and solve problems that arise.
- Interacts with residents and families in a considerate, helpful and responsible manner to ensure a positive and professional environment. Recognizes problem situations and acts to diffuse or appropriately channel to responsible Department Managers.
- Assists residents and family members regarding Veterans benefits and long-term care insurance policies
Actively markets facility and its programs and services to a variety of referral sources by developing and maintaining contact for the purpose of referrals in order to meet budgeted census and mix.
- Develops and maintains favorable relationships with social service departments of hospitals, physicians and their staff, placement services and families for the purpose of referrals.
- Visits referral sources on a regular basis to inform them of the programs and services available at facility and to encourage them to refer potential residents.
- Develops a referral source file and maintains records of follow-up and contacts on a consistent basis.
- Initiates and coordinates the visit of individuals and groups such as social workers, placement counselors, physicians, interested individuals, families, etc., for the purpose of conducting comprehensive tours of facility. Maintain a continuous flow of potential residents is referred and facility consistently exceeds budgeted census.
Evaluates potential residents referred to facility for admissions.
- Within 24 hours of receiving a referral, conducts an onsite visit to evaluate the person for admission.
- Utilizes good assessment skills, completes all required documentation, and prepares the resident for admission, re-admission and/or discharge to the facility. Including orders, medications and assembly of patient chart. Exceeds
- Completes an Assessment Evaluation form and follow up to determine when the person will be available for placement within facility. Conducts follow-up evaluation at least weekly to determine changes in the person’s condition. In addition to seeing the person weekly, maintains frequent phone contact with the referral source. Accurate reports on the status of the potential residents are given to the Executive Director daily.
- Makes every effort to place all referrals at facility by discussing questionable potential residents with the Executive Director, Assistant Executive Director and Resident Care Manager.
- Follows up with all new residents within 24 hours of admission to assess whether needs are being met and the new resident is adjusting to the new surroundings. Consistently interacts with Resident Care Manager and new resident to ensure a positive adjustment to facility and satisfaction on the part of residents and families.
Participates in the planning, organizing and implementing of facility’s Public Relations functions.
- Assists in the planning and execution of Public Relations programs or special marketing events sponsored by facility.
- Participates in local community organizations such as Chamber of Commerce, Health Care Associations, Professional Organizations, Senior Citizen Groups, etc., for the purpose of enhancing facility’s reputation of excellence and encouraging referrals. Seeks out new opportunities to represent facility by active participation in the community and/or professional activities or events.
- Acts as communications source between facility and the community communications network in order to project a positive image of this facility.
