POSITION SUMMARY

Responsible for directing the development, implementation, supervision and ongoing evaluation of the care sites activities program. This includes: the implementation and/or directing/delegating the completion of the activities component of the comprehensive assessment; Contributing to and/or directing/delegating the contribution to the comprehensive care plan goals and approaches that are individualized to match the skills, abilities, and interests/preferences of each resident/tenant. Directs the on-going activities program to support residents/tenants in their choice of activities, both facility-sponsored group and individual activities and independent activities, designed to meet the interests of and support the physical, mental, and psychosocial well-being of each resident/tenant, encouraging both independence and interaction in the community.

Essential Functions Statement(s)

•Develops, assures implementation, supervises and evaluates the effectiveness of the activities program designed to meet need identified in comprehensive assessments of the current resident population.

•Responsible for coordinating and directing the Activities program to meet the specific needs of each resident.

•Ensures residents'\tenants’ lives are enhanced to the fullest extent possible by providing individual and group activities.

•Planning and scheduling of individual activities, small and large groups as defined in State and Federal regulations.

•Maintain documentation as required by State and Federal regulations.

•Must have excellent communication skills with the ability to encourage, form relationships and communicate with residents, families and staff.

•Ability to supervise activities staff and coordinate a variety of programming, and to evaluate residents' responses to activities according to their individual needs.

•Act as or assign Activities staff to be the Designated Staff Person for Resident's Council meetings and forward written Resident's Council suggestions and grievances per Policy and Procedure.

•Attend scheduled meetings as a member of the Interdisciplinary Team of LB Homes.

•The ability to think in ways that produce something new or which lead to novel ideas.

Education & Experience

Licensed or registered, if applicable, by the State in which practicing and is: Eligible for certification as a therapeutic recreation specialist or as an activities professional by a recognized accrediting body on or after October 1, 1990: OR has 2 years of experience in a social or recreational program within the last 5 years, one of which was full-time in a therapeutic activities program; OR is a qualified occupational therapist or occupational therapy assistant; OR has completed a training course approved by the State.
